01 NDIS

Self-managed & plan-managed participants

We currently accept NDIS participants who are self-managed or plan-managed, with registration as a provider underway.

02 Aged Care

Support at Home referrals

We deliver physiotherapy to aged-care clients as an approved allied health partner for aged-care providers across Sydney, under the Support at Home Program.

03 DVA

Gold & White Card holders

We treat DVA Gold Card holders for all clinically necessary care, and White Card holders for their accepted conditions — no gap fee.

04 Private / Self-Funded

Direct, no funding required

Don't have NDIS or a Home Care Package? You can book directly and pay privately — no referral or funding approval needed.

Agape Mobile Health was built on a simple idea: quality physiotherapy shouldn't depend on being able to get yourself to a clinic. "Agape" — a Greek word for unconditional, selfless love — is the standard the practice is named for: care that meets people where they are, without conditions attached. As the practice grows, that same standard will guide every clinician who joins the team.

Frequently asked questions

Yes — we currently accept self-managed and plan-managed NDIS participants. We're not yet a registered NDIS provider, so agency (NDIA-managed) funding isn't available yet, but that's a goal we're working towards.
Yes. Gold Card holders are covered for all clinically necessary physiotherapy; White Card holders are covered for their accepted conditions. No gap fee applies for either.
If your package is managed through a registered aged-care provider, ask them to engage us as your physiotherapist. If you'd rather not go through your package at all, you're welcome to book directly and pay privately instead.
